Neglect contributed to death of much-loved dad found 'unresponsive' in bedroom in locked psychiatric ward, inquest finds

Neglect contributed to the death of a much-loved dad-of-two who was found 'unresponsive' in his bedroom on a locked psychiatric ward, an inquest jury has found.

Niall Tyrrell, 28, fondly described by his mum as the 'life and soul of the party' had been sectioned under the Mental Health Act and was living at Park House, a mental health hospital in Crumpsall run by the Greater Manchester Mental Health NHS Foundation Trust. It is currently rated as 'overall inadequate' by the CQC.

Niall was on 15-minute observations when he was found 'unresponsive' by an untrained support worker in his bedroom, which had been locked from the inside, in the early hours of May 2 2022.

Following a two-day inquest, a jury heard that Niall had a history of depression and complex mental health needs, including a diagnosis of Obsessive Compulsive Disorder (OCD) and Emotionally Unstable Personality Disorder (EUPD) and that he had been moved to an acute ward, called the Mulberry Ward, at Park House on April 9 2022.

The inquest had heard that weeks later, on April 21, Niall was able to escape the locked ward by falsely telling staff his mum Joanne was outside waiting in the car to collect him. Giving evidence, Joanne told jurors she had already warned staff he could be 'crafty' - and had also previously told them that she could no longer facilitate home visits.
