Man, 52, found dead just six days after he was released from prison

A 52-year-old man was found dead just six days after being released from prison.

Karl Armstrong had been jailed for 12 years for various offences including armed robbery, the Liverpool Echo reports. He was released on licence from HMP Buckley Hall to live at Bunbury House Approved Premises on May 17, 2022.

After reporting that he was 'under threat from unidentified sources', two days later, he was moved to Merseybank Approved Premises. On May 23, he was found unresponsive in his room there.

Armstrong, from Birkenhead, suffered from a history of substance misuse and mental health problems. An inquest concluded his death was 'drug-related'.

A new report by the Prisons and Probation Ombudsman found a night security worker at the Merseybank Approved Premises hostel failed to fulfil his responsibilities to the expected standard. He was suspended and subsequently resigned from his position.

Armstrong was moved to Merseybank after reporting he was under threat. He again told staff he was under threat on May 21, 2022, and barricaded himself in his room.

He eventually let staff into his room and told them he was feeling paranoid. The police confirmed Armstrong had not reported anything to them and there was no intelligence to suggest he was under threat.

Armstrong, whose medical records showed he had been diagnosed with post-traumatic stress disorder and suffered from night terrors, also told staff he could hear people talking on his phone and that it had been hacked. On May 22, police arrived at the premises after Armstrong reported a firebomb exploding outside his window.
