Concern as item used in man's suicide 'available on Amazon'

A coroner will write to Amazon after the same product a man used to take his own life was found on the retailer's website. Coroner Victoria Davies has shared her concerns after investigating the death of Adrian Gallagher.

The 24-year-old died at Hollins Park mental health hospital, in Warrington. Ms Davies, area coroner for Cheshire, noted from Mr Gallagher's inquest last month that he had a history of mental health struggles from 2013 until his death four years later.

He was admitted to Hollins Park on June 12, 2017, as an informal patient and discharged four days later. But he was sectioned and re-admitted to hospital on June 17.

The coroner noted he was allowed periods of unescorted leave from hospital, and on November 9, 2017 he returned from a period of leave. Tragically, he was found dead the following morning due to an 'intentional' overdose, and Ms Davies concluded Mr Gallagher's death was suicide.

In a warning that other lives could be at risk, Ms Davies has written a prevention of future deaths report to the Department for Health and Social Care, the National Crime Agency and the Department for Science, Innovation & Technology. She warns that the item Mr Gallagher used to take his own life 'is available to anyone to purchase online, directly from the company', and without age and ID checks until after purchase.
